Seeing the ambulance, Jim, Boyd, and Randall run out to help get Tabitha and Henry out, though From’s sinister monsters are closing in on them. While trying to run back to the bus, Randall sees the cicadas again and is stopped in the road. Boyd tries to start up the ambulance to drive the group away, but realizes that the nurse creature has the keys. She offers a deal: the keys in exchange for Randall. Boyd hesitantly agrees, which not only instills him with massive guilt, but also sets up a huge confrontation given Randall may not actually be dead.

Boyd Sacrificed One Life To Save Others In From Season 3
Boyd’s decision to take the keys while giving Randall over the monsters gives him an ethical dilemma in the same vein as the famous “Trolley Problem.” The tough choice that Harold Perrineau’s From character makes is based on the idea that he’s willing to sacrifice the life of one person in order to save a larger group of people. If Boyd hadn’t taken the keys, then the night might have ended with Tabitha, Jim, Henry, Randall, and himself all dead. Or, he could take the risk of letting Randall die to give the others the best chance of survival.
To be fair, Boyd didn’t have much time to think through this decision. Randall being surrounded by monsters meant it already looked like he was in a position where he couldn’t be saved, and time was of the essence to get those in the ambulance inside Colony House. Boyd’s hesitation proved that giving up Randall for the ambulance keys wasn’t an easy choice, and he knew at that moment that he would be left to deal with the guilt of abandoning Randall.
Randall doesn’t really get along with the other residents in From ’s town, has secluded himself on the bus, and hasn’t been much of a team player.
Furthermore, Boyd likely reasoned that saving Tabitha, Henry, and Jim over Randall was the right choice for the community. Tabitha and Jim have two kids who need their parents, and Henry has his son Victor in town. Meanwhile, Randall doesn’t really get along with the other residents in From’s town, has secluded himself on the bus, and hasn’t been much of a team player since his trauma with the music box monster in From season 2’s ending. That doesn’t mean that Randall’s life isn’t as valuable as the others, but if forced to make that decision, it explains Boyd’s reasoning.

Boyd Projects His Own Guilt About Sacrifing Randall Onto Acosta
After getting inside Colony House, Boyd quickly projects his guilt about leaving Randall onto Acosta, the new police officer who fled the ambulance after the paramedics’ deaths. Acosta was being pursued by the creatures as she ran toward Colony House, and while trying to shoot the many monsters surrounding her, she unintentionally fired a shot inside the house. While doing so, Acosta accidentally shot and killed Nicky inside. The residents of the house were clearly distraught, and Boyd running in to reprimand her only made tensions higher.
Following the paramedics’ deaths, Acosta and Henry are the first brand-new residents of From ‘s town since the bus arrived in the season 1 finale.
Taking his anger out on Acosta indicates that the monsters are starting to get what they wanted after killing Tian-Chen and letting Boyd live in From season 3, episode 1. The monsters wanted to break Boyd, rattle him, and keep him from being the glue that holds the community together. Boyd lashing out also suggests that he knows the monsters’ actions are getting to him. This time around, the monsters didn’t even kill Randall in front of him while he was helpless; arguably worse for his stability and well-being, they forced him to choose to let the monsters kill Randall.

Since Boyd had agreed to let the monsters take Randall in exchange for the keys, he had already come to terms with the notion that Randall wouldn’t survive. However, the end of From season 3, episode 4 shows that Randall has been pinned to the front of the ambulance by the monsters. He’s still alive at that moment, but Randall’s face has been ripped apart and he’s badly injured and bleeding. Therefore, it seems From’s monsters didn’t actually kill Randall, but left him maimed and in critical condition to torment Boyd with the knowledge of his choice to abandon him.
